A 14-month-old girl with quadriplegic cerebral palsy, failure to thrive and profound developmental delay presents with a two month history of episodic back arching and dystonic posturing of the upper limbs. Episodes occur several times daily, most often in the evening when she lies of the floor to watch television after her nasogastric tube feed. She has a previous history of neonatal seizures which the family reports were caused by hypoxic ischemic injury at birth, and were thought to have resolved.
