ANSWER:

Formula: wt. x (desired H b - actual Hb) x 3.0 cc.
= 4 x (12 - 5) x 3 cc = 84 cc
Explanation:
PRBC means Hct of 66%, whole blood Hct of 33%, transfusion of 2 cc RBC/kg increases Hb 1 gm/100 cc
2 cc RBC present in 3 cc of PRBC, so 3 cc/kg PRBC increases Hb 1 gm/100 cc, increases Hct 3.
